As well as the finer holistic details, your personalized end-of-life plan will include the medical and non-medical care you would like to receive before, during, and after your death. We will identify (and complete, when possible) the paperwork needed to allow your death in alignment with your wishes.

Please allow 1.5  to 2 hours for the initial visit. If we need more time, we will arrange additional sessions (no extra cost). I will compile your end-of-life plan into an easy to read document, and email it to you for review. We will connect again to discuss any changes or clarifications you'd like to make.
